Investigation Summary

Investigation Nr: 202312161
Event: 10/26/1999
Employee injured in fall from roof

Employee #1 was working with the employer and a coworker. Employee #1 was standing on an open sided shed roof, some 12 feet above grade, while the other two workers were actually on the roof above him, attempting to remove the chimney. As it was being moved, some pieces of debris fell into Employee #1's face. He reportedly said something to the effect of: "Hey, this stuff is getting into my eyes". Employee #1 stepped backward a few steps, and turned to walk away. He took several steps forward, and stepped off the roofs leading edge. Employee #1 fell approximately 12 feet to the ground. Employee #1 sustained several broken ribs, a punctured lung, and other substantial body trauma and was hospitalized.
