Investigation Summary
At about 2:00 a.m., an employee was gathering garlic samples from the running conveyor line she was assigned to. She inserted her arm into a small space, which houses a large unguarded dip takeup pulley. She was wearing company issued raingear, in a large size, with long floppy sleeves as the work environment is quite cold. As she secured the needed samples, her sleeve became entangled in the nip-point of the pulley, and her arm was pulled in. She fractured her arm and was hospitalized with her injuries.
