Investigation Summary

Investigation Nr: 202311783
Event: 04/30/1999
Employee Loses Finger In Punch Machine

Employee #1, an experienced punch press machine operator, lost one finger and broke another when he failed to use the provided pull backs on a Bliss punch press. He was loading a part on the press with his left hand, operating the press with his foot, and removing a part from the press with his right hand. His rhythm became out of sync, and his hand was still in the press as his foot activated the press. As a result, one finger was amputated and another finger was broken. He failed to use the pull backs to stop the press. He went to a local hospital for treatments.
