EMPLOYEE RECEIVED CUTS WHEN HE FELL INTO AN ELEVATOR SHAFT

On July 16, 1998, Employee #1 entered the first level of a building in search of a couple of pieces of wood. Employee #1 was disoriented as a result of moving from the bright sunlight to a dimly lit area. Employee #1 passed between two studs into what he thought was a room, but was an elevator shaft which had been covered by a piece of plywood. The plywood failed to hold the weight of Employee #1 and he fell 14 feet. Employee #1 was hospitalized for cuts and lacerations.
