Investigation Summary
On December 14, 1998, Employee #1 was called to repair or adjust a blanket folding machine in the laundry facility. After making repairs, Employee #1 had the operator use the folder to see how it was working and found that it needed some final adjustments. The folder had to be running for Employee #1 to adjust some air jets to ensure its proper operation. Employee #1 was standing to the right side of the folder about half way between the operator's position and the ejection end of the folder. The blanket being run through the machine for the test did not quite fold correctly and Employee #1 reached over to make a finite adjustment. At that point, the blanket caught his hand and pulled it under an air bar and safety guard into the creasing rollers fracturing the employee's wrist and arm. The employee was hospitalized with his injuries. There should have been an extension tool to use in the vicinity of the moving machinery and Employee #1 should have removed the bar guard before making adjustments.
