Investigation Summary
At approximately 2:00 p.m. on October 16, 1998, a carpenter's apprentice was in a man-basket on the forks of a Grade All 634. The basket was secured to the back guard and the employee was holding on to the back railing of the basket. The forks of the Grade All touched the ground, causing the basket to tilt toward the back guard of the lift and crushing the employee's left thumb between the back guard and the back railing of the basket.
