EMPLOYEE'S HAND STRUCK BY MACHINE PART

At approximately 7:45 a.m. on March 12, 1998, Employee #1, who worked as a machine operator, was assisting another employee in changing a blank on forming machine Number 184. While doing that, the invert arm reverted back and struck Employee #1's hand and ring finger. Employee #1 was immediately transported to the St. Ross Hospital for treatment. The forming machine was not locked-out by another employee, as instructed.
