At approximately 8:30 a.m. on October 2, 1998, Employee #1, a 32-year-old assistant Flexo operator, lost his hand in an unguarded machine. He was working in the Finishing Department on a Flexo Folder Gluer. The Model Number of the Gluer was 2800. His hand was pulled into the feed rolls. His employer manufactures cardboard containers. His employer did not inform Employee #1 of the process change that left a portion of the feed roll unguarded. His employer had a training program that provided training for most machinery and equipment. However, his employer failed to ensure that proper training was given to Employee #1.
