Hand Amputated-Feeding Containers Into Unguarded Roller

At approximately 8:30 a.m. on October 2, 1998, Employee #1, a 32-year-old assistant Flexo operator, lost his hand in an unguarded machine. He was working in the Finishing Department on a Flexo Folder Gluer. The Model Number of the Gluer was 2800. His hand was pulled into the feed rolls. His employer manufactures cardboard containers. Employee #1 was running an order where the cardboard was automatically fed into the prefeeder. When his left hand became caught in the feed roll, the automatic feed roll became unguarded. His employer did not inform Employee #1 of the process change that left a portion of the feed roll unguarded. His employer had a training program that provided training for most machinery and equipment. However, his employer failed to ensure that proper training was given to Employee #1. Employee #1 was treated for his amputation at a local hospital.
