EMPLOYEE FALLS FROM UNGUARDED PLATFORM

At approximately 7:30 a.m. on February 23, 1998, Employee #1 was working as a team leader on the new car assembly line, Group Code PACO4C, for New United Motors Manufacturing Inc. He had been working for them for 13 years. He was called to a work station and informed that the wrong radiator had been installed in a car on the line. He retrieved the correct radiator and when he returned, the car had moved along the line and was up on the overhead carrier. He went up onto the carrier to make the replacement. He had almost completed the change when the car came to the end of the carrier and he fell off approximately 6.54 ft. He was taken to Washington Hospital and hospitalized. At the location where Employee #1 fell there were no guardrails and he was not wearing any fall protection equipment. Also, he did not stop the line in order to change the radiator.
