Summary Nr: 170722474 | Event: 10/01/1995 | Burn - Electrical Fault In Switchgear | ||||
Two electricians were burned in separate accidents while working in an ITE main switchgear. (Employee #1 was burned on October 1, 1995. Employee #2 was burned on October 2, 1995.) Each was removing 100-ampere and 50-ampere circuit breakers at the time of his accident. Based on investigation of burned hardware in the switchgear cabinet and in the circuit breakers, investigators concluded that a loose connection in the hardware and the elevator circuit breaker screws caused phase-to-phase electrical faults. The ensuing electric arcs burned the employees and caused fires. The 480-volt switchgear was worked energized though the loads were disconnected. The employees were working without rubber insulating gloves and safety glasses. They were each hospitalized with serious burns on their faces, arms, and hands. | ||||||
