Employee's Back Is Lacerated in Fall Into Mixer

At approximately 11:50 p.m. on February 21, 2006, Employee #1 was cleaning and sanitizing mixers 9 through 12. Employee #1 was standing on a lower grate platform using water hose with nozzle to rinse the mixers. He removed the top grate from mixers 11 and 12 and continued rinsing residue from the bottom of the mixers and around the belt and pulley drive. While in the process of turning the power back on to the mixers and putting down the hose to go on break, he tripped over the hose and fell through the unguarded running drive belt and onto the floor. Employee #1 was hospitalized for treatment of lacerations on his upper back, lower back and buttocks, and stitches to the back of his head.
