Employee's Fingers Amputated When Caught in Machine

At approximately 8:45 a.m. on April 21, 2006, Employee #1, of Quality Assurance, was checking the tile line for breakage when he noticed that tiles were being broken at the deburring machine area. He notified a coworker and they started looking in the area for the problem. Employee#1 stooped down on one knee in front of the deburring machine and opened the unsecured door to the bottom chain drive. He extended his left hand and placed his fingers onto the chain to check for tension when the machine cycled, catching four fingers and pulling them into the sprocket. Employee #1 was hospitalized with mangled and amputated fingers.
