Employee's Finger Amputated in Chute

At approximately 1:30 p.m. on October 19, 2005, Employee #1, a carpenter, was attempting to remove cement stuck in a 1-yard concrete bucket. The bucket was filled and attached to a Skytrak rough terrain forklift. It was lifted to drop the cement into an 8-ft by 8-ft elevator wall form. Employee #1 reached into the chute opening with his left hand. The forklift operator closed the gate, without warning, onto Employee #1's hand, amputating four fingers. Employee #1 was hospitalized were his fingers were reattached. However, due to an infection they were amputated.
