Employee Amputates Finger in Machine

On August 3, 2005, the employee, who was a supervisor, stopped and de energized or disengaged the power source for the Lettuce Washer Separator during a cleaning operation. The failure to stop or deenergized the machine resulted in an amputation of the left hand middle finger. The employee was hospitalized and treated for amputation of the finger.
