Investigation Summary
At approximately 8:30 a.m. on July 8, 2004, Employee #1 was operating a Toyota forklift to relocate equipment in the storage yard area. He attempted to lift a 36-in. diameter steel auger bit weighing approximately 270 lbs from a metal rack. He positioned the forks underneath the bit and lifted it approximately 12-13 in. off the ground, dismounted the forklift, and picked-up a 5 pound sledge hammer with his right hand and started striking the rack several times to dislodged the auger bit from the rack. The auger bit became loose and move downward on the forks, pinning Employee #1's left index finger between the bit and forks. Employee #1 was using forks for braced while hitting the rack. He pulled out his hand, leaving his glove and the severed tip. Employee #1 was hospitalized and treated for amputation.
