Investigation Summary
On March 8, 2004, Employee #1, a carpenter with CBC Framing, Inc., was working on a job-built scaffold that was secured to a structure by 2-in. by 4-in. members nailed to the rim joists. The scaffold was about 9.5-ft-high, had a span of about 6 ft, and had no guard rails installed. He was taking measurements for a corbel to be installed when the scaffold collapsed. Employee #1 was hospitalized at San Jose Medical Center. A coworker indicated that he and Employee #1 were not using fall protection. The scaffold was not erected under competent supervision and it is unclear if the scaffold was erected using good engineering practice.
