Employee killed when pinned on scissor lift

At approximately 11:30 a.m. on May 23, 2002, Employee #1 was found unresponsive on an elevated scissor lift by a field engineer, apparently pinned between its railing and a sprinkler pipe on the second deck of an 8-floor parking garage. When the lift was lowered, Employee #1 fell inside it. Efforts to revive him were unsuccessful and he died. A coworker nearby when Employee #1 was found indicated that Employee #1 was working fast and would catch up. Equipment failure was ruled out as a causal factor. Training documentation indicated that Employee #1 had received training on the operation of the equipment.
