Employee trips, falls, and dies

At about 1:20 a.m. on October 12, 1999, Employee #1, a 38-year-old warehouse worker, suffered head injuries and compound fracture to his left arm when he fell from an order picker-type lift. Employee #1 was operating a Model 537 Raymond turret truck while in the process of pulling orders from a storage racking system. Unable to reach product or some plastic wrapping, Employee #1 disconnected his safety lanyard and attempted to climb out of the lift. Stepping out onto the rack, he tripped, lost his balance, and fell 17-feet 4-inches landing on the concrete floor. The cause of the accident has been determined to be a misjudgment of hazard by Employee #1. Employee #1 was hospitalized and treated for his injuries.
