Investigation Summary
Employee #1, a 31 year old male, had been assigned to the Rodgers 505 vertical panel saw,. Employee #1 was trained and authorized to operate the vertical panel saw. On the day of the injury, Employee #1 approached the vertical panel saw to trim a piece of wood. He placed the wood against the table and the gate. He started the saw without noticing the saw head location. The saw blade came out from behind the table and struck the wood, pushing it against the guard bar. Employee #1 attempted to grab the wood and his fingers came into contact with the saw blade, amputating two finger tips. Besides inattention on the part of Employee #1, the machine was not maintained in a safe operating condition according to the operations manual. According to the manual the guard bar was to move into place against the table before the saw blade raised above the table. If the guard bard had been adjusted correctly, the guard would have been in place holding the wood against the table and there would not have been exposure to the rotating blade. Employee #1 was not hospitalized for his injuries.
