Investigation Summary
Employee #1, a painter, fell from a work scaffold. The platform had no guardrails at the door opening. The edge of the platform was more than 16 in. from any wall and was 8-ft-4-in.high. Employee #1 sustained six cracked ribs, a cracked shoulder blade and a fractured skull. The employer had not erected the scaffold, nor inspected the work site before their employees were allowed to access the platform. Employee #1 was hospitalized.
