Investigation Summary

Investigation Nr: 170838668
Event: 10/15/2003
Employee's finger amputated in hydraulic conduit bender

At approximately 2:07 p.m. on October 15, 2003, Employee #1, an apprentice electrician was working with a journeyman electrician. The journeyman electrician was setting up the bender to bend an offset in a 3-in. metal conduit using an Enerpac B-400 hydraulic conduit bender. The shoe in was not in the machine and Employee #1 was sent to the gang box to get it and told to insert it in the shoe. The components were not aligned, and the pin would not go all the way in, so Employee #1 wiggled it with his right fingers to get it fully inserted. The journeyman found the conduit was not properly positioned to bend and backed off the cylinder. The protruding pin hit the binder frame and crushed Employee #1's right index finger between the pin and the frame hydraulic conduit bender, pinching off the bone to the joint and crushing the flesh. The employee was not hospitalized.
