Employee Amputates Fingertips in Conveyor Belt

At approximately 10:30 a.m. on March 18, 2004, Employee #1 was working at the top of a conveyor in an aggregate batch plant, where he had removed a guard to a pulley assembly two days earlier to replace belts. The belt pulley is 8 inches in diameter by 36 feet apart, 0.5-inch side grooved by 2 inches at belts. While the conveyor belt and pulley assembly were running, he inadvertently set his hand on the belt, which pulled his right hand fingers into the pulley. The tip of Employee #1's right ring finger was amputated and the right middle finger of the same hand was gashing. Employee #1 was hospitalized for his injuries.
