Investigation Summary

Investigation Nr: 170838999
Event: 12/19/2002
Two Employees Caught in Conveyor

At approximately 3:30 p.m. on December 19, 2002, Employee #1 and Employee #2 were cleaning a moving conveyor belt at a lumber mill. The accident occurred when Employee #2 was sweeping sawdust and chips below an elevated operational conveyor belt when his raincoat sleeve became caught in the in-feed side of the conveyor belt roller mechanism. Both hands, then both arms became entangled and Employee #2 was lifted off his feet and suspended 3 feet off the ground with his arms rolled up and pinched in the conveyor/roller and his head against the moving belt. Employee #1 was injured when he came to help Employee #2 at the time the belt was being reversed by the machine operator. The reversal of the belt ultimately ejected the arms of Employee #2, drawing in the hand and arm of Employee #1, and fracturing the left forearm of Employee #1, who was pushing on the arms of Employee #2. The two employees were taken to Howarth Memorial Hospital in Willits, CA where Employee #1 was hospitalized for surgery on his left forearm to repair the fracture. Employee #2 was admitted for observation of multiple contusions including those to his head.
