Investigation Summary
On October 21, 2002, Employee #1 was involved in a filtering operation, which was located outside and in a covered area. A Quadra press with a diatomaceous earth as a filtering media was used. Employee #1 was working from the top of a 10-ft-high wine filter and did not wear fall protection. He fell down and sustained cervical strain and contusion.
