Employee's hand injured in screw machine

At approximately 7:20 a.m., on March 2, 1993, Employee #1 was shutting down a Nitride briquetter in anticipation of scheduled maintenance. While stepping down approximately 8 in. to a landing, Employee #1 lost his balance. In an effort to keep from falling, Employee #1 reached out with his gloved left hand and grabbed an approximately 10 in. area of the screw machine that was unguarded. His hand was pulled into the screws. He pulled his hand out and shut off the screw machine. Employee #1's left index finger was partially severed, and the heel of his left hand was severely lacerated and fractured. Employee #1 has been the sole operator of the briquetter for five years.
