Employee catches arm in vial making machine

Employee #1 was operating an Ambeg RP glass vial making machine when she reached into the machine and caught her right arm while trying to remove glass fragments. Employee #1 sustained nerve damage and burns to her right hand and arm. The employer didn't have a lockout/tagout program to prevent employees from placing their hands in the machine. Employees wear eye protection and gloves.
