Tip of employee's thumb amputated in mechanical power press

Employee #1, a press operator, had set up a press (manufactured by Standard Machinery Co.) that has a full revolution clutch. He was doing a small job lot with the press when a piece got stuck in the die. As he tried to remove the piece with his right thumbnail, the press started chattering and tripped again, amputating the tip of his right thumb. The press is equipped with a safety pull back system, but Employee #1 stated that he was busy that day rushing around and did not think to put on the pull backs. Employee #1 has 13 years experience and is trained.
