Investigation Summary
Employee #1 was removing a clogged 4 in. cast iron sewer pipe from a 23 ft long by 6 to 7 ft deep trench with vertical side walls. A side wall collapsed, burying Employee #1 to his chest. His helper jumped into the trench to dig him out but the fire department arrived and asked the helper to get out. Employee #1 was then talked through digging himself out. Once freed, Employee #1 was hospitalized. The employer has no work rules or policies regarding work in excavations.
