Event: 04/08/1992
Employee knocked unconscious when struck by forklift

Employee #1 was bending over and marking conduit in an aisle where forklifts travel. Another worker drove a Clark forklift, serial # 52109, into the aisle and while making a left turn apparently struck Employee #1 in the head, causing him to fall backwards into a scissor lift and leaving him unconscious. The forklift was moving four chipper shredders, which were stacked two on two. The forks were 3 inches above the floor. The forklift operator had received training and was a licensed operator with 10 years of experience. The forklift had been inspected that morning and all alarms, horns, and lights were in working order. Employee #1 was not protected from moving vehicles and had been working in this area for a 1/2 hour.
