Employee injured in fall from bridge beam

Employee #1, a steelworker, was working approximately 32 ft above the river shoreline on a bridge expansion project. He was bolting up a splice plate on a 36 in. I-beam with an approximately 5 1/2 in. flange, using a safety belt as a work positioning tool to reach the bottom outside bolts. The anchorage point was a metal spud bar inserted in the top bolt hole and the safety lanyard doubled around the anchorage point. As Employee #1 rose from finishing the job, the spud bar came free and he fell approximately 32 ft, landing partially in the water. Employee #1 sustained a fractured leg and was hospitalized. He had approximately four years steelworking experience, but inadequate hazard training. The employer had an inadequate safety and health program. No safety net was provided.
