Employee killed in fall from moving panel

Employees were working in electrical installation. Employee #1 placed two panels over the beams as a platform to stand on. The employee placed a wood portable ladder over the panels to reach the work area. The panels were not fixed. When the employee finished the work, he came down the ladder and stepped on the panel's edge. The panel revolved, causing the employee to fall 40 ft. He died.
