Investigation Summary
At 1:30 p.m. on October 17, 2016, an employee was unloading a "mini-dumpster" from the forks of a powered industrial truck into a trash compactor located in the facility parking lot. The employee placed himself between the front of the powered industrial truck and the compactor with the forks in a slightly elevated position. The parking brake was not functioning adequately. The truck rolled forward, pinning the employee between the mini dumpster and the compactor; crushing his abdomen. The employee was killed due to crushed internal organs.
