Investigation Summary
At approximately 10:00 p.m. on May 14, 2005, Employee #1 was struck in the head by a container that was struck by a forklift. He then fell to the floor and struck his head. He died two days later from his injuries. Further investigation revealed that the forklift operator was not looking in the direction he was traveling, was traveling at a high speed, and did not use the horn just prior to the accident.
