Employee killed when crushed by metal tubes and racks

Employee #1 wanted material stacked on racks four tiers high; however, the forklift truck driver cautioned that this was against the rules. The wheel broke on the trolley carrying the tubes on the racks. Employee #1 was crushed and killed by falling tubes and racks.
