Investigation Summary
At 8:15 a.m. on August 4, 2016, a worker was installing insulation consisting of 4 feet wide by 20 feet long metal panels to the roof. While working, the worker unhooked his lifeline from its anchor point and fell approximately 36 feet to the concrete ground and was killed as a result of head and body trauma. The incident was witnessed by the foreman onsite.
