Investigation Summary

Investigation Nr: 170574024
Event: 09/25/1996
EMPLOYEE KILLED IN FALL

Employee #1, a temporary laborer, was removing debris from the bottom of a ship by placing the debris in five-gallon buckets and hoisting them to the deck of the ship. Employee #1 was standing on a platform that was 20 ft below the deck and was ready to hoist a bucket that was full. Employee #1 was holding the rope that was tied to the bucket. Another employee on the deck to receive the full buckets looked into the hatch because it seemed that it was taking longer than usual. The other employee stated he saw Employee #1 shaking and trembling and his eyes had rolled inside the eye socket and all he could see was the whites of Employee #1's eyes. He said he then saw Employee #1 fall backwards over the rail to the bottom of the ships hold. Employee #1 died.
