EMPLOYEE LOST 3 FINGERS ON RIGHT HAND. THE POSSONS PULL-OUT DEVICE WAS NOT PROPERLY ADJUSTED TO PREVENT OPERATOR'S HAND FROM BEING IN POINT OF OPERATION AT TIME OF DIE CLOSURE.
