Investigation Summary
Employee #1, a laborer, was working in a 19-ft-long by 9.83-ft-wide by 6.67-ft.- to 7-ft-deep trench. A backhoe had made a hole between two concrete piers measuring about 5 ft across and about 5 ft apart. Employee #1 and coworkers were scooping dirt into the hole until enough had accumulated for the backhoe operator to scoop out. The operator brought the backhoe bucket into the trench about 2 ft from the bottom. The operator swung the bucket left about 10 feet to the east end of the trench, pinning Employee #1 against the foundation of the adjacent building. He was killed.
