Event: 04/16/1997
One employee killed and one injured when knocked off ladder

Employees #1 and #2 were using 80 lb of air pressure to test the return chilled water line for leaks. The line was equipped with a multipurpose valve that functioned as a check valve. Once the air went through the check valve it could not be release; the valve would not let it exit even when the employees released the air at the manifold, where there was a shut-off valve and an air gauge. Employees #1 and #2 started to remove the 8 in., 35 lb cap that had been installed on the chilled line for the test when it blew off. They were knocked from the 10 ft stepladder on which they were standing. Employee #1 was killed and Employee #2 sustained injuries, but was not hospitalized.
