Employee's arm amputated in meat auger

Employee #1, a new employee with a subcontractor cleaning company, was manually scrubbing a discharge auger when the machine was re-energized. His right arm was amputated above the elbow, and he was hospitalized. Employee #1 had failed to lockout or tagout the machine prior to starting work.
