One employee killed, two injured in fall from platform

On November 11, 2003, Employee #1 and a coworker were working from a platform that was hoisted by a rough terrain forklift. The platform fell from the forks, dropping Employee #1 and the coworker 22 feet. Additionally, a coworker, standing below, was struck by the platform as well. Employee #1 died and his coworkers were injured, with one suffering a serious facial laceration and the other receiving superficial cuts and bruises.
