Employee killed in fall through roof

Employee #1, age 20, was part of a crew that was re-roofing a structurally unsafe roof. He was walking across the old portion of the metal roof where the purlins and sheets had been cut for removal. When Employee #1 stepped on a cut sheet, it flipped up and he fell approximately 38 ft through the roof to the floor. He sustained a fractured skull and was killed. The workers had not been trained to recognize hazards. Employee #1 was wearing a full body harness at the time of the accident, but he was not tied-off.
