Employee killed in fall from ladder

Employee #1 had placed the top half of an aluminum ladder on a drive-in to climb 13 ft to the roof. The bottom of the unsecured ladder kicked out causing Employee #1 to fall approximately 12 ft to the asphalt. He broke his shoulder and struck his face on the ladder. Employee #1 died from the fall.
