Employee injured when struck by boom attached to forklift

At approximately 8:00 a.m. on October 6, 1991, Employee #1 and a coworker were attempting to change a large circular saw. Employee #1 attached a long jib boom to the forks of an 8,000 lb forklift. The boom was raised above the saw, putting the upright mast at its full lift height. The coworker got on the forklift and lifted the saw from the framework. He then attempted to lower the saw to the floor. The upright became bound in the up position. Not seeing that the upright was bound, the coworker continued to lower the hydraulic cylinder until the chain went slack. He then tilted the upright forward until the saw rested on the floor. Employee #1 went to disconnect the saw. The pressure was relieved from the upright and the boom fell. The boom missed Employee #1 on the way down, but rebounded back up, striking him in the chest and head. He was knocked to the floor, where he sustained an injury to the back of the head. First aid was administered and he was transported by ambulance to Roseburg Hospital, where he remained in critical condition.
