Employee injured when struck by concrete barricade pieces

Employee #1 was working in the bottom of an approximately 17 ft deep trench formed by an excavation wall and the walls of the structure. Barricades weighing approximately 2 tons each were installed at the top edge of the excavation. A barricade was tipped into the excavation when another employee attempted to pick up a barricade with a forklift. The forklift forks caught on an inside barricade and tipped it into the excavation. The concrete barricade broke into chunks upon striking the rock bolts and broke over the employee. Employee #1 was found lying between two chunks of barricade. He was flown out by Life Flight and hospitalized for multiple fractures.
