Employee injured when struck by clay in trench

Employee #1 was in a trench preparing to connect a water pipe. The trench was 6 ft deep by 15 ft long by 7 ft wide at the top and 3 ft wide at the bottom. It was benched back 2 feet on each side. A boulder (clay) kicked out and struck the employee. He sustained a broken jaw and foot, and underwent colon surgery. Other employees nearby didn't see the accident.
