Investigation Summary
Employee #1 was near the unguarded edge of a roof doing repairs, 12 ft above a concrete floor. He walked within 1 ft of the roof's edge to speak to a coworker on a different roof top. When the conversation ended, Employee #1 took a step and fell over the edge. He sustained massive head injuries. Employee #1 was transported to a local hospital in extreme critical condition. His condition worsened until he died approximately 5 1/2 days after the accident.
