Employee injured when struck in the chest by steel rod

At approximately 2:30 p.m. on January 10, 1994, Employee #1 was helping guide a P-80 rod pusher, model LP-80, serial #2K0332, horizontally under a road to install an 8 in. pvc water pipeline. A 1 3/4 in. by 4 ft piece of steel rod, being used to hold an 8 in. clevis in place, rotated out and delivered a glancing blow to Employee #1's chest. He was hospitalized.
