Employee killed when head is crushed by mold stand

Employee #1 was operating a tire mold stand. He raised the turret (top) and with the top in the UP position, he and a coworker put their heads into the mold to verify an ID number that was stamped on the inside of the mold. When Employee #1 located the number, the coworker took his head out of the mold. When he did, he heard a crack and the turret fell on top of the mold, trapping Employee #1's head and killing him.
