Employee's fingers mangled in rotating shaft

At approximately 12:30 p.m. on November 22, 1991, Employee #1, a glass furnace off loader, was working the glass furnace wearing rubber gloves when his hand was caught by an unguarded rotating shaft. Three of his fingers were mangled. He required hospitalization.
